Joining AMVI/Prospect Health Network simply starts with CalOptima. The enrollment process differs based on benefits that you are applying for. These two distinct processes are more clearly defined below:
Medi-Cal and Healthy Families Participants
Step 1
Once it has been determined that you are eligible for Medi-Cal or Healthy Families benefits, you will be mailed a packet from CalOptima instructing you to choose a Primary Care Physician (PCP) from a directory listing and indicate your choice of PCP and affiliated Network on the enrollment form. Look for a doctor affiliated with the AMVI/Prospect Health Network [PHC#027] that meets your current geographical and language needs.
In order to assist you in finding out additional information regarding your choice of doctors in the AMVI/Prospect Health Network, you may use our website tool which supplements any information you may receive in your enrollment packet from CalOptima. Simply click on the Find a Doctor link to see a list of participating providers.

Once you have selected your preferred doctor, complete the enrollment form as required and return the required information to CalOptima. You will receive a membership ID shortly thereafter indicating the physician of your choice and the corresponding health network.

- Direct Referral by your physician to the most commonly used specialists so you don't have to wait for an authorization
- We provide tools to our physicians to identify those individuals that have not yet obtained the recommended screenings for preventative health, such as immunizations, mammography, colorectal screenings, diabetes and cholesterol management so you and your family stay healthy
- Customer Service Representatives dedicated to assisting you with any healthcare issues you may have including appointment scheduling for your convenience
- Care Plus Program for those members requiring hospitalization to ensure that once you leave the hospital you are on a fast track to wellness
- Health and Wellness Classes to assist you in controlling your diabetes, coping with stress, weight management for children and adults, just to name a few
- Transportation Coordination for those members who do not have an ability to obtain their healthcare services as result of transportation limitations
